COVID-19 Booster Clinics (Pfizer or Moderna Available) at the Middletown Senior Center (Monday, April 11th or Tuesday, April 19th)

The FDA and CDC recently authorized a 2nd booster for certain individuals.

 

People who are 50 and older and received their 1st booster at least 4 months ago are now eligible to receive a 4th dose of either Moderna or Pfizer vaccine.

 

  • MASKS REQUIRED for this health service
  • Please stick to your designated appointment time so that there isn't a backup of people or shortage of parking.
  • Note: It must be at least 4 months since your last booster shot
  • You will need to sit for 15 minutes after your booster shot
  • You can choose Moderna or Pfizer (both will be available).  The formulation of the boosters is the same as your original booster.  1/2 dose booster for Moderna and full dose booster for Pfizer.

Please bring:

  • Your COVID-19 vaccination card
  • A completed consent form.  If you don't have the ability to print this ahead, please arrive 10 minutes early to fill out a form in the billiard room at the senior center.  Consent forms can be downloaded from middletownct.gov/seniors

  • Tuesday, April 12th from 9am-12pm and 1pm-4pm at the Durham Activity Center, 350 Main Street 2nd Floor
  • Wednesday, April 6th from 10am-1pm at the Cross Street AME Zino Church, 440 West Street
