    Site Plan and Narrative must be attached to this application at the bottom of the form. Insurance Certificate can also be attached (only required if using public property)

    Must show the following: • Outdoor Dining/Retail Area layout and dimensions • Location of tables with number of seats noted and all furniture. • Location of fencing/barriers • Building(s)/structures dimensions • Staff stations and pathways • Sidewalk dimensions • Entrances/Exits • All other features or obstructions (lamp posts, trees, etc.) • Property owner’s approval (if applicable) • Temporary heaters, sanitizing stations, lighting, signage and fire extinguisher locations
  8. Brief Narrative Complete*
    Narrative should include at a minimum the following: hours of operation, general description of outdoor activity area needed and if it will be on public property, public safety measures to protect customers as well as if alcohol will be served in the outdoor area.
  9. Insurance Certificate Complete*

  10. Property Owner Approval*
    Please submit authorization by the Property Owner if you are not the owner of the parcel.
