Primary Series (1st and 2nd doses):

  • Moderna for ages 12+

Bivalent Boosters:

  • Moderna bivalent vaccine for ages 12+ 
  • Pfizer bivalent vaccine for ages 12+
  • Must be at least 2 months from the last COVID vaccine

Middlesex Health

By appointment

  • Pfizer vaccine available for people ages 5+
    • Children ages 5-11 will receive the pediatric dose
    • People ages 5-17 must be accompanied by a parent or guardian
    • Government-issued proof of age is required; see website for details
  • Moderna and J&J 1st and 2nd doses for ages 18 and up
  • Pfizer boosters for ages 12 and up
  • Moderna and J&J  for ages 18 and up
    • Must be at least 5 months since the last Pfizer of Moderna dose
