Seasonal Flu Vaccine Clinics

The health departments from Cromwell, Durham, and Middletown are partnering with Stop and Shop pharmacy to bring flu vaccine clinics to our communities!  

How to Get Vaccinated: 

Attend any of our clinics in Cromwell, Durham, or Middletown in September, October, or November. No residency is required. Masks and social distancing will be enforced at all clinics to protect our patients, staff, and community. 

Bring your insurance card and photo ID. Most people can get vaccinated at no cost through their insurance. 

We welcome all people ages 12 and up to get the flu vaccine. Age restrictions may apply for other vaccines.

Consent Form

If you have access to a printer, print the consent form ahead of time to bring to the clinic. If you do not have access to a free printer, don't worry! Consent forms will be provided at the clinic. You MUST bring your current insurance card to the clinic to receive a vaccine.
